Description about this resource: If you have already completed a Volunteer Screening Form in the past and you have been asked to renew your background check, please use this form. If this is your first time to complete a Screening Form, please use the generic Apex Kids Volunteer Screening Form. We must update the screenings every 2 years for the safety of our children. Thanks for your cooperation! Please print this form, fill it out completely, and return it to the Apex Kids desk in the kids lobby if you are interested in serving with Apex Kids.
